Supervisor of Clinical Services
Position is 40 hrs/wk, Monday-Friday with on-call responsibilities

Responsible for overseeing the day to day operations and clinical functions of the home health nursing team. Coordinates professional assignments and scheduling, coordinates care of the multi-disciplinary group providing care to patients, monitors clinical documentation including notes, orders, and OASIS review. Knowledge of OASIS and Home Health regulations care critical to this position. Works closely with Agency leadership to develop and implement education programs and competency.

Minimum Requirements: RN and 3 years of nursing experience, at least 1 of which must be in home care setting.
Preferred: MS and prior supervisory/management experience.

RN CASE MANAGER- ASSISTED LIVING (VERNON)
Non-Union—20 hrs/wk, weekdays 8:00am-1:00pm, Monday-Thursday with required on-call rotation

Provides nursing care to clients with chronic and stable conditions within an assisted living facility. Provide direct supervision of the CNA staff in the absence of the Manager. Daily responsibilities include medication management, aide supervision and client assessment. Must communicate regularly with the professional staff of the ALSA team and the assigned RN on call. Coordinates health care services for clients living in their own homes, located within an assisted living facility. Daily integration of ALSA services shared with direction from the client/family, physician, home care, hospice and out-patient services. Under the direction of the primary care physician the RN case manager will manage medications, perform assessments, provide direct care, plan aide services and is involved in teaching of client and family.

REQUIRED: Current license to practice as an RN in CT. Demonstrated experience of a minimum of 1 year direct supervision of CNA staff. Current knowledge and hands on experience with elder medication management. Experience working with elders with complex medical and psychosocial needs. DESIRED: Experience providing direct client care in an Assisted Living Facility. Experience working with a Managed Residential Community.

RN Cottage Designee
The RN Cottage is responsible for the overall supervision of nursing care in the Cottage in the absence of the SALSA.

Essential Job Duties:
  1. Assists in managing all assisted living nursing personnel in the delivery of nursing of nursing services and the provision of activities of daily living.
  2. Assist's in developing all agency objectives, standards of care, policy & procedures concerning nursing services.
  3. Assists in program planning, budgeting and evaluating activities related to the clinical services provided.
  4. Oversees the Wellness Program, assuring biweekly wellness checks are completed and resident issues are addressed.
  5. Meets with families to review service plans
